Bird Flu is caused by Influenza A virus. The main causes of bird flu outbreaks are poor biosecurity, contaminated feed and water.
Wild birds are acting as carriers

Avian influenza or bird flue is a highly contagious viral disease caused by Avian influenza virus (H5N1). The waterfowls are the major reservoir of bird flu. Direct contact with the infected waterfowl, contact with the surface contaminated with Avian influenza virus, infected poultry house equipment, shed employee & also contaminated feed truck may spread bird flu.

I think that majorly four factors contribute to the spread of bird flu. They include:
Wild Bird Migration; birds like the wild waterfowl spread avian influenza along migratory routes, contact with Infected Birds; direct contact with infected birds or their secretions spreads the virus. Also live Bird Markets; crowded markets can facilitate virus transmission among birds, and finally the movement of man and equipments- personnel like farm workers, vehicles, and tools can carry and spread the virus.
